Maternity self-referral

Since May 2020, it has been possible to refer yourself to a midwife at the Princess Anne Hospital using their online form. You simply need an email address & web browser to complete the process.

You must be at least five weeks in to your pregnancy. After you complete the form online, they will send you a confirmation email to let you know they have received your form. Please allow up to 10 working days for them to process your booking.

Once your form has been processed, your information will only be shared with the people caring for you. You'll be contacted by their Triage Midwife to discuss your referral and let you know the next steps. They aim to do a full booking appointment in week 10 or 11 of your pregnancy. They will also tell the surgery that you are pregnant.
